| Spa Amenities |
| All Spa services include the use of Spa robes,
slippers, towels, lockers, and showers. Bodylines is not responsible
for loss or damage of personal property. Please leave valuables
at home. |

| Arrival Time |
| Please arrive 20 minutes prior to your first
appointment so that your session can begin on time. Starting your
session on time allows you to receive the full benefit of your
complete and unhurried service. So that the following client is
not inconvenienced, appointment duration will be adjusted for
late arrivals at full fee. |

| New Clients |
| During your initial visit, you will need a
few additional minutes prior to you first appointment to fill
out a brief questionnaire. You will receive a guided tour of our
Spa and amenities and complimentary refreshments. |

| Contraindications |
| For health and safety reasons, any client with
heart or circulation problems, high blood pressure, diabetes,
pregnancy, or any other health issue should consult with their
physician before receiving any Spa treatment or using the Dry
Sauna. It is not recommended that pregnant women receive body
wraps or any treatment that involves excessive heat. |

| Earn Day Spa Points towards discounts on treatments.

| Gratuities |
| A customary 15% gratuity will be added to all
Spa Parties of 4 or more people with multiple services. Our Spa
therapists pride themselves in delivering excellent service. Gratuities
are graciously accepted and appreciated. |

| Cancellation Policy |
| Because your space is reserved for you, a 24
hour notice of cancellation is required or full fee is due. Full
fee is due for cancellations on "same day" appointments. A credit
card guarantee or gift certificate number is required on all appointments.

| Refund Policy |
| Pevonia Skin Care - Cash refund available
with receipt within 30n days of purchase on unopened product.
Store credit will be issued on open product with receipt within
30 days.
General Merchandise - Cash refund on undamaged/unopened
merchandise with receipt within 30 days.
Gift Cards - Transferable but not refundable.
